What You’ll Need

Referral

A valid referral from your GP or specialist is required for your appointment and to claim a Medicare rebate.

Medicare Card

Please bring your Medicare card (and private health insurance details if relevant).

Medical History

Include a list of current medications and any previous test results such as ECGs, echocardiograms, or blood tests.

Questions or Concerns

Write down any questions or symptoms you’d like to discuss so we can address them during your visit.
